Staples High fall sports tryout information

In order to be allowed to try out for a fall sport, every athlete must have on record a current health history and physical examination before he or she can participate in the sport throughout the season.

The physical examination and history must have been completed within the past 12 months. Please use the HAR-3 Rev 4/2012 to fulfill that requirement.

Form C Emergency Information form is also mandatory for participation. Forms are available in the Staples High School main office, guidance, health or athletic offices, as well as on the school's website at http://shs.westport.k12.ct.us/athletics/athletic-forms. The completed forms must be approved by the nursing office or the school's athletic trainer prior to tryouts.

Athletes will not be allowed to participate in tryouts or practices without approved forms.

Please drop off or mail the completed forms by Aug. 10 to the health office. During most days in August, a health assistant will be in the health office to receive forms, but because of schedule variability, it is best to call 203-341-1230 before coming, or mail forms to:

Sophomores, juniors and seniors need to bring their June 2013 report card to the first tryout.
